EVALUATIONS All new patient evaluations are conducted by Board Certified Specialists with expertise in the Diagnosis, Evaluation and Treatment of Painful conditions.

LOCATION Our office is a first floor, handicapped accessible location in the Lakewood Medical Office at the intersection of Capitol of Texas Highway (Hwy 360) and Lakewood Drive. The address is 7307 Creekbluff Drive, Austin, Texas 78750. Free ground-level parking is provided.

OFFICE HOURS Our office hours are Monday through Friday, 8:00 a.m. until 5:00 p.m. After-hours answering is provided by the Medical answering service who will assist you in reaching our physicians in the case of an emergency or to leave a message for the office.

OFFICE VISITS
On your first visit, you should bring with you the Patient Information forms typically mailed to you in advance. Otherwise you are asked to arrive 15 to 30 minutes early to complete the forms. The information you provide will allow us to treat you effectively and to assist with filling your insurance claims. You should also bring your insurance card, related previous medical records and x-rays or imaging films. Medical records are held in strictest confidence and will not be released without your permission. Please inform us of any changes in address, telephone numbers, insurance coverage or medical concerns.

APPOINTMENTS Our first responsibility must be to address the needs of the patient in front of us and some times urgent or complicated cases delay appointments. Every effort is made to maintain our schedule. We ask for your understanding when delays are unavoidable, and if necessary, we will gladly reschedule your appointment so as not to unduly inconvenience you. Please notify the office at least 48 hours in advance if you cannot make a scheduled appointment.

TELEPHONE CALLS We receive a large volume of telephone calls every day and these are answered by an attendant or message system. Please briefly relate the purpose and need of your call. If our staff is busy with other patients, your call will be returned as soon as possible. Our personnel are trained and prepared to respond to the majority of calls regarding your medical needs. Calling repeatedly will only delay our ability to respond to all calls and is discouraged.

PRESCRIPTIONS Prescription refill requests are to be made when you are in the office for scheduled appointments. We retain the right to charge a handling fee for prescription refills made outside of office appointments / visits as we have to manually pull and review the record to validate the refill. It is $15 for each such refill request and may not be covered by insurance and is your responsibility. Class 2 or "triplicate" refills are to be obtained by coming in each 4 weeks for a "medcheck" where we will validate your med need and request. These are set at 28 days such that the rx will never be empty on a weekend. Refills are not processed on Fridays, after hours, or on weekends. If you have not seen the doctor during the past twelve months, you will need to schedule a follow up appointment in the office prior to consideration of any prescription refills.

INSURANCE We are preferred, specialty providers in a number of managed care plans
such as Affordable, Anthem, Private Health Care Systems, PruCare / PruPlus, Blue Choice, Health Select, Seton Physician Health Network, etc. Most of these plans require an authorization referral from your primary care physician before seeing our specialists. If you are a member of such a plan, we will be able to see you only after you have obtained such a referral. We are also participating providers for Medicare. We will file insurance claims for you to any of the plans in which we participate; however, deductibles and co-insurance payments are required at the time of service. For those patients not covered by our participating plans, payment in full is expected at the time of service. If for any reason this is not possible, please let us know in advance so that you can speak with our Patient Accounts Staff to make other arrangements.

PAYMENT For your convenience, payment may be made by cash, check, Visa, Mastercard, Discover or American Express. In situations requiring special financial considerations, alternative payment plans must be approved prior to services being rendered.
